Use this form for submitting the following client requests to the PayNW Sales Team:

•  Change of Legal Entity (new ownership)
• Adding new EIN
• Adding Modules with PEPM charges (PR, TLM, HR, Benefits, etc.)
• Changing pay frequency other than Bi-Weekly/Semi-Monthly changes
